On behalf of the company, I certify that we are familiar with and agree to abide by the Interstate Commerce Act, and Recodification thereof, pertaining to the payment of Transportation and related charges. I hereby grant permission to the above credit references to release pertinent information regarding our accounts to Special Service Freight Company. Furthermore, my signature attests to the financial responsibility, ability, and willingness of the Company to pay all transportation and related charges, and commits the Company to pay within 30 days of the invoice date. A monthly administrative fee of $25 and interest of 1.5% per month will be charged on all overdue accounts. The Undersigned authorizes and release all bank persons, finance agencies and companies listed on this application and authorize the checking of credit. The undersigned agrees to pay all collection costs, court costs and legal fees incurred to collect delinquent balances.
